1. Acceptance of Terms – By using the Gift Card, the holder agrees to these terms. The issuer may update these terms at any time without prior notice.
2. Eligibility and Redemption – The Gift Card is valid only at the clinic where it was purchased for the health screening package specified on the front of this card. It cannot be exchanged for cash unless required by law. Issuer reserves the right to cancel this card if it has been tampered with or if the serial number hasn’t been activated in the respective system/database.
3. Non-Transferable and Non-Refundable – The Gift Card is non-transferable and cannot be refunded. Lost, stolen, or damaged cards will not be replaced unless at the issuer’s discretion.
4. Limitations on Use – The Gift Card is valid only for 1 health screening service and cannot be used for other purchases like medications, products, etc. It may not be combined with other offers unless stated. Additional tests performed that are not part of the health screening package will incur additional costs at the time they are conducted.
5. Appointments – The Gift Card does not guarantee an appointment. Bookings are subject to availability, and advance scheduling is required.
5. Validity Period – The Gift Card is valid until June 30, 2026
6. Governing Law – These terms are governed by the laws of the country this card was purchased in. Any disputes will be handled by arbitration under these laws.
7. Modification and Termination – The issuer may modify or end the Gift Card program at any time, in line with applicable laws.
